Media Consent Form - ADULT - YMCA Robin Hood Group

By submitting this form, I agree to the following:

I give my permission to YMCA Robin Hood Group to use pictures/audio/videos of myself in general publicity, public and internal facing campaign materials and funding bids with YMCA Robin Hood Group, the charity’s branch YMCAs (YMCA Greater Nottingham, YMCA Newark and Sherwood, YMCA York and East Riding etc.), and partner charities The Zone Youth Project and The Malt Cross Trust. Consent will last for three years from the date below.

This content may also be shared to partners and stakeholders of YMCA Robin Hood Group who have been deemed as necessary for delivering services, such as funding bodies and delivery partners. The purpose of this is to support ongoing funding and delivery of core charity services.
I give permission for these organisations to receive and share the media content captured to their own marketing platforms. YMCA Robin Hood Group retains ownership of this media and can request removal by third-parties at any time.
After three years, neither YMCA Robin Hood Group nor the organisations mentioned above will use the image(s), audio or video without seeking further consent from me.
YMCA Robin Hood Group and the other organisations mentioned above may share my (please tick all that apply).

I understand that the photos/videos/audio could be used on the above mentioned organisations’ websites, online social media channels (including but not limited to Facebook, Twitter and Instagram), printed promotional materials such as posters and flyers, and funding bids of the charities mentioned above.

I understand that I can remove my consent regarding images and/or marketing communications at any time by emailing the YMCA Robin Hood Group Marketing and Communications team on mpr@nottsymca.org or by calling 0115 9567600 and asking to speak to the Marketing team. YMCA Robin Hood Group will confirm updates to my consent record via my preferred contact method within 20 working days of receiving my notification.
