Social Worker (Nottinghamshire)

The post holder will be a qualified Social Worker who can assess people with complex needs under the terms of The Care Act, contribute to the setting of personal budgets and create and implement support plans. Your support will be critical in helping people achieve a healthy and independent future. Your approach will be characterised by a focus on the person – ensuring at all times that their wishes and dignity are advocated for.
You will collaborate with a range of professionals and organisations across Nottingham and Nottinghamshire to identify exactly how a social care package of support might meet someone’s needs, co-ordinate activities and achieve the very best outcomes. You will consider the attached level of risk while promoting independence.

The post is funded by the National Lottery Community Fund and the European Social Fund (ESF). The post holder will be working on the Opportunity and Change (OAC) project supporting its delivery partners and participants. The project is part of a national programme called Building Better Opportunities.
