YMCA Greater Nottingham is growing, and supporting more people than ever to live healthy, independent lives. We’re looking for someone who can support people with multiple complex needs to access the services to which they are entitled. You will measure your success in this role by your ability to support people from a position of need and reliance to a healthy and independent future.

The project is part of a national programme called Building Better Opportunities and the post is funded by the National Lottery Community Fund and the European Social Fund (ESF). The post holder will be working on the Opportunity and Change (OAC) project supporting people facing severe and multiple disadvantages to become socially and economically included, through access to education, training and employment.

Your day to day will be characterised by the development of bespoke action plans – defined by the individual needs of the people you are working with. You will work with a number of other agencies, and with staff involved in supporting your clients to coordinate their activities so that they achieve the very best outcomes.

You will keep an eye on the bigger picture too. You will support your colleagues to manage people with multiple complex needs as part of the YMCAs commitment to continuous improvement of our services and personal development. One of your key responsibilities will be the monitoring and evaluation of the personal navigation service with a focus on the quality and impact of the support on offer.

Salary: £22,287 per annum plus benefits
